Theme of Guilt and Healing
Episode 5 rolls out to be the chapter of guilt, healing, and finally, romance. The reward for all the negative aspects we’ve seen so far. Doctor on the Edge episode 5 starts with Ji-ui’s sad memories of Seonu’s funeral. As he goes deep into the water, Ji-ui’s mind thinks about the haunting day when he was saved, but his friend apparently died.

We can clearly see the theme of guilt right from the start. Inside the cold demeanour of Ji-ui hides a man who no longer wants to cause anyone any distress. Not only Ji-ui, but we also see other cast members enduring guilt. Say, Ju-cheon blaming himself for not knowing Deok-hwa’s health condition, Ha-ri feeling guilty for always putting herself and others in dangerous situations and so on.
We see Ju-cheon’s storyline making an effect. After Deok-hwa’s incident, he is afraid of treating other patients, believing he might end up hurting them. Medical dramas usually focus on life-saving heroic moments, but this series delves into the personal struggles of such heroes. How even the god-like doctors make blunders. When Ju-cheon eventually treats Nurse Eom, we can see he is healing.

Speaking of Nurse Eom, her conversations with Ju-cheon are one of the episode’s charms. They both bring lots of warmth and humour to the plate. We can see how she literally exaggerated her side effects to make Ju-cheon prepare himself to handle a patient. So sweet! Also, we can surely see them becoming a couple sooner.

Romance Between Ji-ui and Ha-ri – Turning Point
Ha-ri seems to be enduring too many rumours. We even see how the nurses gossip, and her aggressive ex-colleague makes her nightmares come true. We can see why the island became a safe zone for her through scenes of her past. We can see how genuine Ji-ui is whenever he hears these rumours and keeps his composure. He never gossips or asks Ha-ri about all these things. Instead, he defends her and stays beside her.
We also get tiny, cute scenes such as the failed attempt of Ha-ri trying to wipe ketchup from Ji-ui’s face (ah, the tension), them holding hands on the ferry, sharing music and small gestures of kindness.

Granny Mija’s Heartbreaking Storyline
The revelation about Mija’s breast cancer spreading to her brain raises the emotional tension. Her diagnosis impacts the entire island and the people. We can see how the islanders weren’t aware of the pain she was hiding. It shows that they do care about her, but she cared about them more than she hid her issues to keep them all active around her.

The dinner scene between Ha-ri and her grandmother is one of the episode’s emotional highlights, capturing the fear and helplessness that often accompany serious illness.
The Confession
Ji-ui, on witnessing Ha-ri crying in Chi-yeon’s arms, finally realises he loves her. So he musters up the courage and confesses his feelings for her. Ha-ri’s smile acknowledges it, and the two share their first kiss. The kiss felt rewarding to the viewers because it didn’t feel rushed but earned. The two went through ups and downs together, and now they’ve decided to seal their pact together.

Final Thoughts
Episode 5 may not feature the adrenaline-fuelled medical emergencies of previous chapters, but it excels at developing the characters. It balances all the essential aspects such as romance, humour, sad storytelling, and unity. This by far is one of the series’s strongest episodes. The kiss, of course, headlines the episode, but beneath that, it is the honesty of the two that captures the viewers.
A heartfelt and rewarding episode that deepens every major relationship while finally giving viewers the romantic payoff they’ve been waiting for.
